What Is Tirzepatide?

A Dual-Action Weight-Loss Injection

Tirzepatide (brand name Mounjaro) is a once-weekly injectable medication that activates two hormone receptors: GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide). This dual action makes it the most effective injectable weight-loss treatment currently available.

In the SURMOUNT-1 clinical trial (NEJM, 2022), patients taking 15mg weekly achieved average weight loss of 20.9% over 72 weeks. Some participants lost more than 25%. It also significantly improves blood glucose, making it particularly effective in type 2 diabetes or insulin resistance.

Who Is Tirzepatide For?

Clinical Indications

  • BMI ≥30 (obesity) - weight management
  • BMI ≥27 with weight-related comorbidity (hypertension, T2DM, sleep apnoea, PCOS)
  • Type 2 diabetes - blood glucose management
  • Insulin resistance and metabolic syndrome
  • PCOS with obesity or insulin resistance
  • Patients who plateau on semaglutide (Wegovy)

Dosing

How Tirzepatide Is Started

Dosing principles:

  • • Usual starting dose: 2.5 mg once weekly
  • • After at least four weeks, may be increased to 5 mg once weekly
  • • Further increases in 2.5 mg increments only after at least four weeks at the current dose
  • • Recommended maintenance doses: 5 mg, 10 mg or 15 mg once weekly (maximum 15 mg)

Dose escalation depends on individual clinical need, response, adverse effects and tolerability — this is not a fixed, automatic schedule. Your consultant will agree the right pace with you.
